A non-refundable payment of $1,000.00 (U.S.), payable to IMPAC, Inc., must accompany all completed applications. The IMPAC™ Mailorder and Internet Pharmacy Accreditation Standards will be faxed to the survey contact person upon receipt of the completed application, fee and signed non-disclosure agreement.

The survey must be scheduled within ninety (90) days of receipt of the application. Partial payment of the survey fee, $10,000.00 (U.S.), is due 30 days prior to the survey date. The balance of the survey fee and reimbursement for the surveyors’ travel expenses is due before receiving accreditation report.

IMPACIMPAC is Trade Marked surveyors will require the following on-site:
impac Room that provides privacy to review documents
impac Documents labeled and organized by IMPAC standard
impac List of all employees with their credentials and/or job titles for selection of the random Human Resource files.
   
IMPACIMPAC is Trade Marked Accreditation Status Designations:
1. Full Accreditation: 100% compliance with the standards.
2. Conditional Accreditation 75% compliance to standards with no consumer safety issues can be changed to full status if evidence is provided that all standards are met within 90 days of the Survey Findings Summary.
3. No Accreditation: Organizations with less than 75% compliance or with serious consumer safety issues or that do not submit evidence of compliance within 90 days of the Survey Findings Summary may re-apply for survey after a 6-month waiting period.
       
IMPACIMPAC is Trade Marked Accreditation Term is 2 years
Organization changes during the IMPAC™ Accreditation Term.
impac In the event an organization adds additional pharmacies or call centers or websites to their operations IMPACIMPAC is Trade Marked must be notified within 30 days of the change(s).
impac Pharmacies or call centers and websites added to an organization during the accreditation term must be surveyed for compliance to current IMPACIMPAC is Trade Marked standards within 90 days, if they are going to display the IMPACIMPAC is Trade Marked seal.
impac Failure of a pharmacy or free standing call center to inform IMPACIMPAC is Trade Marked of organizational changes will lose accreditation status, and will not be eligible to apply for accreditation for a period of three years.
impac Failure of an organization to maintain IMPACIMPAC is Trade Marked Mailorder and Internet Pharmacy Standards or found to have falsify information given to IMPACIMPAC is Trade Marked is at significant risk to lose accreditation status.
