Spa Standards:
IMPAC Spa Standards are designed to meet the various types of Spas and services
provided by Spas. Services determine the applicable standards, not the square footage or size of the Spa. A Spa
will be surveyed according to category of Spa and the type of services that the Spa provides. The categories of
Spas are as follows:

| Day Spa/ Destination Spa: |
A Day Spa or Destination Spa provides wellness and beauty services that do not
require physician oversight and direction. A Day Spa or Destination Spa may provide what is
classified as traditional spa services such as massage, pedicures, manicures, facials,
microdermabrasion, and Reiki. |
| Medi-Spa: |
A Medi-Spa provides wellness and beauty services where some of those services
require physician oversight and direction. Examples of services that require physician oversight and
direction are laser treatments, chemical peels and advanced microdermabrasion. |
| Physician Practice Spa: |
A Physician Practice Spa or Medical and Wellness Spa provides both medical and
health procedures and wrap-around spa services. Examples of Physician Practice Spas are a cosmetic
dermatologist’s and/or plastic surgeon’s practice with the services of a medical esthetician.

Upon receipt of the Application and the signed Non-Disclosure
Agreement, IMPAC, Inc. will determine the category of the Spa and the applicable IMPAC Spa Standards. The IMPAC
Spa Standards will then be sent to the Spa.
Scheduling a Survey:
The survey must be scheduled within ninety (90) days of receipt of the application. A conference call with IMPAC
staff to review the Spa Standards and to answer any questions may be scheduled by the Spa. |

$9,000 to $15,000 ** (USD) |

**Fee is dependent on scope of invasive procedures and anesthesia

Survey pricing for multiple spas corporation or network, consultations or
preparation audits are available upon request. If any spa is a hybrid of two categories of spas, the fee schedule
and applicable standards will be adjusted to match the services offered.
Payment of half of the survey fee is due 30 days prior to the survey date. The balance of the survey fee and
reimbursement for the surveyors’ travel expenses is due before the final accreditation report.
* Note: The accreditation of a day spa or destination spa includes an on-site survey and compliance with IMPAC™
Standards. It is more comprehensive than the Gold Star List.

Full Accreditation: 100% compliance with the standards |

Conditional Accreditation: 75% compliance to standards with no client/guest safety
issues can be changed to full status if evidence is provided that all standards are met within 90
days of the Survey Findings Summary. |

No Accreditation: Organizations with less than 75% compliance or with client/guest safety issues or that do not submit evidence of compliance within 90 days of the Survey
Findings Summary may re-apply for survey after a 6-month waiting period. |

Loss of facility license |

Failure of the organization to notify IMPAC, Inc concerning changes in services that
would move the Spa to another category. A change in spa category may require an on-site survey of new
services. |
