selection of the random Human Resource files. |

IMPAC
Accreditation Status Designations: |
| 1. |
Full Accreditation: 100% compliance with the standards. |
| 2. |
Conditional Accreditation 75% compliance to standards with no consumer
safety issues can be changed to full status if evidence is provided that all standards are met within
90 days of the Survey Findings Summary. |
| 3. |
No Accreditation: Organizations with less than 75% compliance or with
serious consumer safety issues or that do not submit evidence of compliance within 90 days of the
Survey Findings Summary may re-apply for survey after a 6-month waiting period. |

Accreditation Term is 2 years |
| Organization changes during the IMPAC™ Accreditation
Term. |
 |
In the event an organization adds additional pharmacies or call centers
or websites to their operations IMPAC
must be notified within 30 days of the change(s). |

Pharmacies or call centers and websites added to an organization during
the accreditation term must be surveyed for compliance to current IMPAC
standards within 90 days, if they are going to display the IMPAC
seal. |
 |
Failure of a pharmacy or free standing call center to inform IMPAC
of organizational changes will lose accreditation status, and will not be eligible to apply for
accreditation for a period of three years. |
 |
Failure of an organization to maintain IMPAC
Mailorder and Internet Pharmacy Standards or found to have falsify information given to IMPAC
is at significant risk to lose accreditation status. |
